The following rules and regulations apply to the Customer Satisfaction Online Survey Sweepstakes offered by Heraeus Kulzer, LLC ("Sponsor").
Sweepstakes begins the First day of every month and ends on the last day of every month. The rewards will be handed out once a month from March 1, 2010 till December 31, 2011. ("Sweepstakes Period")
To Enter: Log on to www.heraeusfeedback.com, complete the online survey. Tell us about a product, sales associate or customer service experience. At the end of the survey, you will be asked to fill in your contact information. You may review as many products as your wish and each product review will constitute on entry. Review which contains obscene, lewd or offensive language will be disqualified. Entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. Sponsor is not help responsible for all entries that are lost, late, damaged, illegible or otherwise deficient. Sponsor is not responsible for any typographical or other errors in offer, administration of the award or in the announcement of the prize. Sponsor, in its sole discretion, reserve the right to disqualify any person tampering with the entry process or otherwise in violation of the rules. Sponsor further reserves the right to cancel, terminate or modify by tampering, unauthorized intervention.
Sweepstakes is open to legal residents of the 50 United States, the District of Columbia, Canada and Puerto Rico who are 18 years of age or older as of the date of entry, except employees of Sponsor and its affiliates, subsidiaries, advertising and promotion agencies and their immediate family members.
One winner will be selected in a random drawing on or about the first day of the following month. All decisions of the judges are final. Odds of winning will be determined by the number of eligible entries received.
No substitution, exchange or transfer of price except Sponsor, in which case a prize of equal or greater value will be substituted. By participating, entrants agree to these rules. Entrants are solely responsible for their own actions and agree to hold harmless Heraeus Kulzer, LLC and any participating sponsors, from any liability for losses, damages or injuries arising in connection with their participation in the sweepstakes or the award of a prize. Acceptance of prize constitutes consent to uses winner's name in editorial, advertising, publicity and television broadcast purposes without further compensation except where prohibited. Prize winner need not to be present at time of drawing. Sweepstakes is subject to all federal, state and local laws and regulations. Income and other taxes, if any and all expenses not detailed in the price description are the sole responsibility of the winner. If an email address is provided on entry from, entrant acknowledges that he/she 1)is over the age of 18 , 2) has read the privacy policy statement and 3) is granting Heraeus permission to send them marketing messages.
